Under Support at Home (SAH), care management is tracked separately from other services. The government caps care management fees at 10% of the participant’s total ongoing budget. This article covers how care management works in Visualcare and what your team needs to know.

Care management overview

  • Care management fees are capped at 10% of the participant’s ongoing quarterly budget (excluding short-term supports)
  • The Care Management Pool (CMP) is quarantined for care coordination activities
  • Care Partners deliver and record care management activities
  • Tracking is available at both the individual participant and organisation level

Recording care management time

Use the Add Time feature to log care management activities against a participant. Any notes added through this feature also appear under the Notes tab in the participant’s profile.
Care management logging is currently participant-specific to meet compliance and claiming requirements. A centralised view is available for oversight, but bulk entry across multiple participants simultaneously is not supported at this time.

  • Start guiding your Care Partners on new processes for tracking time and taking notes during care management activities
  • Care management claims are submitted separately, detailing the activity hours and applicable rates
  • Visualcare facilitates logging, claiming, and tracking against the pooled care management amount at both participant and organisation levels
